Beginning therapy can feel overwhelming, but our team is here to make each step clear and comfortable. From your first inquiry through your child’s individualized treatment program, we work closely with your family to provide expert guidance, thoughtful communication, and compassionate pediatric care.
Begin by completing our secure online intake form. This allows our office to learn more about your child, your concerns, relevant medical history, insurance information, and scheduling needs.
After reviewing the information submitted, a member of our team will contact you to discuss appointment availability, required documents, insurance procedures, and next steps.
Submission of an intake form does not guarantee an appointment, insurance coverage, authorization, reimbursement, or payment.
Before your child’s first appointment
Families will be asked to provide:
Completed intake and medical-history forms
Front and back of the child’s insurance card
A physician’s prescription
Completed Financial Policy and Credit Card Authorization forms
Relevant medical, developmental, school, or previous therapy records
